Please bring picture I.D., documented proof of Social Security Number, last year's return as well as any current tax documents.
Registration required. Schedule an appointment by calling the Shoreline-Lake Forest Park Senior Center, (206) 356-1536. Walk-ins will be accommodated as time permits
